What Mass Tort Intake Center collects when you fill in a case review form, why, and who it goes to. It describes what this site actually does, not a template.
What we collect when you submit a form
Your name, telephone number, email address, state and ZIP code, because a case reviewer cannot call you back without them.
Your answers to the questions on the form for the claim type you chose.
Your IP address, and the country and region it resolves to. This is used to check the submission is genuine and to record where consent was given.
The exact wording of the consent notice you were shown, stored with your record along with the date, the time and your IP address. We store the words themselves rather than a reference to them, so it is always possible to show what you specifically agreed to.
Which page you arrived on, where you came from, and any campaign or referral code in the address.
If you were called back, a record that the call happened and notes from it.
What we do not collect
We do not ask for, and the form will not accept, a Social Security number, a bank or card number, or a government ID number. If a page ever asks you for one of those, it is not us.
We do not use advertising cookies or tracking pixels on this site.
Who it goes to
Your details go to Mass Tort Intake Center's case management system, and from there to a law firm or its agent that handles the type of claim you described.
We are paid for that introduction. That is how this service is free to you, and you should know it: our income comes from the law firms, not from you. It does not change what we tell you about your claim, but you are entitled to know the arrangement exists.
Under California and several other state privacy laws, passing your information on in exchange for payment may count as a “sale” or a “share” of personal information, whatever it is called commercially. We would rather tell you that plainly than argue about the definition. You can opt out at any time — see “Your choices” below.
That firm may contact you about your potential claim. Whether they take your case is their decision, not ours.
We use a small number of service providers to run the site: a hosting provider, an email delivery service, a service that checks whether a submission is genuine, and a service that records the consent interaction. They act on our instructions and do not get your information for their own purposes.
We do not sell or rent your information to advertisers, to data brokers, or to anyone outside that chain.
Calls, texts and emails
If you ticked the consent box, you agreed to be contacted at the number and email you gave, including by automated dialling system, prerecorded or artificial voice, and text message.
Consent is not a condition of receiving any legal service, and you can withdraw it at any time. Reply STOP to a text, or ask the person who calls, or email us using the address below.
Withdrawing consent stops the calls and texts. It does not delete a claim already with a law firm — that is between you and them.
How long we keep it
Lead records and their consent snapshots are kept while a claim is live and afterwards for as long as we may need to show that consent was properly obtained. In this industry that is normally several years, because a challenge can come long after the call.
Submissions that are obvious spam are discarded.
Your choices
You can ask what we hold about you, ask us to correct it, or ask us to delete it. Depending on where you live you may have a legal right to those things; we will honour the request either way, unless we are required to keep a record of consent.
You can tell us not to pass your information to a law firm — in other words, to opt out of the “sale” or “share” described above. Doing so means we cannot connect you with anyone, so it usually only makes sense if you have changed your mind about pursuing a claim. Call the number on this site, or tell whoever calls you, and say you want to opt out.
You can opt out of contact at any time without giving a reason. Reply STOP to a text, or say so on the phone.
Residents of California, Colorado, Connecticut, Virginia and other states with privacy legislation have additional rights, including the right not to be discriminated against for exercising them. Exercising any of these rights costs you nothing and does not affect how we treat you.
If somebody is acting on your behalf, they may make these requests for you; we may need to check that they are entitled to.
Children
This form is for adults. Where a claim concerns a child, it is the parent or guardian who fills it in and whose contact details we hold. We do not knowingly collect information directly from anyone under 18.
Security
The site is served over an encrypted connection and submissions are stored on servers we control. No system is perfect, and we will tell you if something happens that we believe puts your information at real risk.
